How many cleanings a year at the dentist? 2 or 3?
The maximum number of cleanings allowed is 3 per year. A year begins on July 1 and continues to June 30 of the following year.

Is there a fee for using EAP services?
No. There are no inital charges to use the EAP benefits. If you choose to continue receiving services, some fees may apply. More information available at http://www.educatorseap.com/

Yes.

Do I need pre-approval for dental services?
No.

Is there a yearly limit on my dental benefits?
Yes. Please to refer to your benefit plan document.

When do I have to submit my claims by before it’s too late?
You can always check with our plan administrator Fitzharris for any deadlines.
Most claims are due by June 30 of the school year you are seeking reimbursement.

What are the annual start/end dates for CCTBF services?
All CCTBF services have a plan year which begins on July 1 and ends the following June 30.
